ADA Standards of Care 2026: What's new for clinicians
The ADA released its 2026 Standards on 8 Dec 2025. Highlights:
• CGM: Recommendation expanded to older adults with type 2 diabetes on insulin. Real-time CGM is now the recognised standard.
• Insulin pumps/AID: C-peptide or autoantibody results are no longer required before starting.
• Targets: A1C <7% and BP <130/80 for most adults. Systolic <120 is encouraged when cardiovascular or kidney risk is high.
• CKD: Wider use of finerenone, and SGLT2 inhibitors continued in advanced CKD.
• Early T1D: People with multiple islet autoantibodies should be referred for presymptomatic staging.
Context: ICMR-INDIAB estimates diabetes prevalence among Indian adults at 11.4% (about 101 million people).
