COPD: it isn’t only a smoker’s disease
COPD caused 3.4 million deaths in 2023, about 6% of all global deaths, and nearly 90% of COPD deaths in people under 70 occur in low- and middle-income countries. Smoking causes over 70% of cases in high-income countries but only 30-40% in LMICs, where household air pollution from biomass or coal is a major driver. Other causes include occupational dust and fumes, childhood asthma, and poor lung growth after prematurity. Diagnosis needs spirometry, so ask about cooking fuel and occupation, not just pack-years.
