World Organ Donation Day | The science behind a second chance at life

Organ transplantation is far more than replacing a failing organ. It is a race involving immunology, critical care, surgery, organ preservation and precise donor–recipient matching.

🔬 A few scientific facts worth knowing:

1. Not every death can result in organ donation.
For deceased solid-organ donation, organs must remain sufficiently perfused and viable until retrieval. Only a small proportion of deaths occur under circumstances that permit organ donation.

2. Compatibility goes beyond blood group.
Depending on the organ, allocation may consider ABO compatibility, body/organ size, medical urgency, geography and immunological factors such as HLA compatibility and pre-existing antibodies. The goal is to reduce the risk of immune-mediated graft injury while finding the most suitable recipient.

3. The recipient’s immune system sees the graft as “non-self.”
T lymphocytes, antibodies and other components of the immune system can recognise donor antigens and initiate rejection. This is why most solid-organ recipients require carefully managed immunosuppressive therapy after transplantation.

4. Time outside the body matters. ⏱️
Once blood supply is interrupted, organs undergo ischaemic injury. Preservation solutions and controlled cooling slow cellular metabolism, but transplantation remains time-sensitive because prolonged ischaemia can impair graft function.

5. One deceased donor may transform several lives.
Organs that may be donated after death include the heart, lungs, liver, kidneys, pancreas and intestine, with a single donor potentially saving up to 8 lives. Tissue donation can benefit many more.

6. The need still greatly exceeds the supply.
WHO reported that more than 150,000 solid-organ transplants were performed worldwide using 2022 data, meeting 10% or less of estimated global need.

Transplantation represents one of medicine’s most remarkable intersections of science, systems and human generosity.
