The Longevity Paradox

We Are Living Longer...Can We Live Better for Longer?

For most of human history, living longer was itself the ultimate measure of progress.

Medical advances, improved sanitation, vaccination, antibiotics, better nutrition and improvements in socioeconomic conditions have dramatically increased human life expectancy.

The global average life expectancy at birth increased from around 46 years in 1950 to more than 70 years in 2023. By 2050, the global population aged 60 years and older is projected to exceed 2.1 billion [1].

But it creates a new question.

What happens to those additional years?

Living longer does not automatically mean living healthier. The expansion of lifespan has not necessarily been accompanied by an equivalent expansion of healthspan - the years lived in good health and with preserved function. One of the papers reviewed here describes an estimated gap of approximately nine years between lifespan and healthspan [2].

For healthcare professionals, this distinction is increasingly important.

The goal of healthy aging is not simply to postpone death. It is to preserve function, independence, physical and mental well-being, and the ability to participate in life for as long as possible [1,2].

That changes the question from:

"How long can we live?"

to:

"How long can we remain capable of living well?"

Healthy longevity appears to emerge from the interaction of biology, behaviour, environment, healthcare and social circumstances [1,2,3].

The First Shift: From Lifespan to Healthspan

The concept of aging has traditionally been associated with biological decline, increasing disease risk and mortality.

The WHO-oriented definition discussed in the public-health literature focuses on maintaining functional abilities that allow people to experience well-being in older age. Healthy aging therefore goes beyond simply being free from disease. It includes the ability to maintain physical, mental and social well-being and to remain independent within one's environment [1].

This distinction matters because aging and illness are not synonymous.

People can reach advanced ages while maintaining substantial functional capacity. Conversely, a person may experience chronic disease, disability or functional decline long before reaching old age [1].

The public-health article highlights the growing burden of chronic disease and multimorbidity as populations age. Longer lives are increasingly accompanied by conditions such as cardiovascular disease, cancer, diabetes and neurodegenerative disorders, while the coexistence of multiple chronic conditions makes care more complex [1].

This creates what could be called the longevity paradox:

Medicine has become increasingly successful at adding years to life, but the next challenge is ensuring that those years are not dominated by disease, disability and loss of independence [1].

The response proposed is to protect lifespan, which raises another question.
